What is physiotherapy after surgery
Physiotherapy after surgery is the work of getting a body that has been through an operation moving safely again. It is not the same as resting. Resting is what causes most of the avoidable problems.
People here say operation ke baad ki warzish. After any significant operation, pain and fear of damaging something make people hold still. Within days that produces stiff joints, weak muscles, a chest that does not clear and a higher risk of clots in the legs.
Your surgeon decides when you may start and what you may do. Physiotherapy works inside those instructions. It never replaces them and never overrides them.
Symptoms you might have
- Swelling around the wound and below it, often worse in the evening and lasting weeks
- A scar that is red, raised and hard at first, softening and fading over many months
- Numbness in the skin around the scar, since small nerves are cut by any incision
- Muscles that feel weak out of proportion to the operation
- Difficulty taking a deep breath or coughing after chest or abdominal surgery
- Constipation and a bloated abdomen in the first week, which moving helps
Why it happens
- Joints stiffen quickly when held still, and are harder to free later than to keep moving
- Muscle begins to waste within the first week of inactivity
- Lying still after chest or abdominal surgery lets the bases of the lungs collapse, which is how chest infections start
- Immobility raises the risk of clots forming in the leg veins
- Swelling that is not managed slows healing and limits movement
- Diabetes and smoking slow wound healing, which delays everything else
When you should see a physiotherapist
The surgeon clears the start. Many operations have physiotherapy beginning on the first day, in the hospital bed. Others need the wound well healed first. The discharge instructions are the authority, so bring them in.
Come as soon as you are cleared. If you were discharged with nothing arranged and you are stiff, weak or afraid to move, come in rather than waiting weeks for a follow up.
Go back to your surgeon the same day if the wound becomes hot, red, swollen or starts leaking, if it opens, or if you develop a fever. Go to a hospital immediately for a hot, swollen, painful calf, with or without breathlessness, which can be a clot, or for sudden chest pain or breathlessness.
How it is treated
The plan depends on which operation you had, so the first session is assessment: what was done, what your surgeon has restricted, how the wound looks, what moves and what has gone weak.
- Exercise therapy, graded from gentle movement to real strengthening as healing allows. This is the main treatment
- Breathing exercises and supported coughing after chest or abdominal surgery, to keep the lungs clear
- Early mobility work, getting in and out of bed, standing, walking and stairs, done safely rather than avoided
- Swelling management with elevation, compression and cryotherapy, applied around the wound and not on it
- Manual therapy to stiff joints and tight muscle away from the operated area
- Scar mobilisation once the wound is fully closed with no scabs
- TENS for pain where it is limiting movement, on intact skin away from the wound
Physiotherapy decides nothing surgical and prescribes no medicine. Ultrasound, electrotherapy, needling and cupping are not used over a fresh or open wound, over numb skin, or where implants make them unsuitable. Exercises, and what to avoid
What helps
- Ankle pumps and deep breathing from the first day, every hour while awake. These reduce clot and chest risk more than anything else
- Supported coughing after abdominal or chest surgery, holding a folded towel firmly against the wound
- Getting up and walking short distances often, rather than one long walk a day
- Gentle movement of every joint you are allowed to move, to comfortable range and no further
What to avoid
- Lifting, straining or carrying beyond the limit your surgeon set, including children and shopping
- Abdominal exercises such as sit ups until you have been cleared, usually several weeks at least
- Stretching or massaging a scar that is not fully closed
- Pushing through sharp pain at the wound. Aching with movement is expected, sharp pulling is not
What you can do at home
Walk a little, often. Several short walks a day do more for your chest, bowels and legs than one long effort.
After abdominal or chest surgery, sit upright, take ten slow deep breaths every hour, and support the wound with a folded towel when you cough. Breathing deeply feels frightening and it is safe. Not doing it leads to a chest infection.
Keep the wound clean and dry as instructed. Once fully closed with no scabs, massage the scar with a plain moisturiser for a few minutes a day, and keep it out of the sun.
Elevate a swollen limb above heart level when resting. A cold pack near the area helps, never over an open wound or numb skin.
Pain relief is your doctor's prescription. Being in too much pain to move is a reason to go back to them, not to lie still.
How long it takes
It depends on the operation, and your surgeon gives you the timeline. As a rough guide, keyhole procedures often return to normal activity in two to four weeks, open abdominal surgery in six to eight, and major joint or ligament surgery over three to twelve months.
Physiotherapy itself is commonly four to twelve weeks of sessions, with home exercises continuing longer. Joint replacement and ACL reconstruction run well beyond that.
Scars keep changing for twelve to eighteen months, softening and fading long after everything else has settled.
Recovery is faster when movement starts early and the home programme is done daily. It is slower with poorly controlled diabetes, with smoking, and when the first weeks were spent in bed.
