TREATMENT TECHNIQUES

Electrotherapy

Electrotherapy explained honestly: what the machines do, why it is an adjunct rather than a treatment on its own, and the contraindications that matter.

What is electrotherapy?

Electrotherapy is not one treatment. It is the name for a group of machines used in physiotherapy that pass a current or another form of energy into tissue: TENS, interferential therapy, muscle stimulation, therapeutic ultrasound, laser and short wave diathermy.

The misunderstanding to clear up first is the big one: lying on a machine is not physiotherapy. Electrotherapy is an adjunct. At best it reduces pain and relaxes a guarded muscle for a few hours, and that is useful, because it is a window in which you can move and exercise. It does not strengthen a weak muscle to normal, it does not heal a disc or a tendon, and on its own it changes very little.

What it can help with

  • Acute neck and low back pain, where relief for a few hours makes gentle movement possible
  • Muscle spasm, where the muscles around a painful spine or joint are locked in a guard
  • Pain that is blocking exercise, where the aim is only to open a usable window
  • A muscle that will not switch on after injury or surgery, particularly the thigh muscle after knee surgery
  • Joint stiffness needing deep warmth before stretching or mobilisation
  • Nerve pain, where TENS is the most useful member of this group and can be continued at home
  • For most long-standing pain, strengthening does far more, and the honest advice is often that you need no machine at all

What happens in a session

Nothing is switched on until you have been assessed, because the machine, the settings and the placement all come from the examination. A pad placement that suits nerve pain down a leg is wrong for a stiff shoulder.

For a current-based treatment the skin is checked and cleaned, pads are placed around or either side of the painful area, and the intensity is turned up slowly while you say what you feel. You should feel a clear, comfortable tingling or buzzing, sometimes a light tapping. It should never be painful and never burn. You stay still for ten to twenty minutes with the switch in reach.

Afterwards the skin may be slightly pink where the pads sat, and that fades. Then the important part: hands-on treatment and exercise, done while the area is more comfortable. That is the whole reason the machine was used.

Does it hurt?

No. Current-based treatment feels like tingling, buzzing or gentle tapping, and heat-based treatment feels like comfortable warmth. The correct intensity is strong enough to feel clearly and still comfortable. Turning it higher does not make it work better.

Speak up immediately if you feel burning, a sharp sting under one pad, or heat that keeps building. That means the setting, the pad or the skin contact is wrong, and continuing risks a burn. Burns from electrotherapy almost always happen because someone stayed quiet, or because the area was numb.

Mild pinkness under the pads is normal and fades within an hour. A blister, a red mark lasting into the next day, or pain that is worse after treatment should be reported.

Who should avoid it

Some of these are absolute, so Dr. Zahra asks about them before anything is switched on.

  • A pacemaker or implanted defibrillator. Currents are not applied, and interferential therapy and short wave diathermy are not used at all
  • Pregnancy. Nothing is applied over the abdomen, lower back or pelvis
  • Over the front of the neck, the eyes or the head, and never with one pad on the chest and one on the back, which sends current across the heart
  • Skin that is broken, infected, inflamed or numb. Numb skin cannot warn you about a burn, so loss of sensation from diabetes or nerve injury rules the area out
  • Active cancer, or any undiagnosed lump or swelling
  • A swollen painful calf or a known clot in a vein, and any area of active bleeding
  • Epilepsy, where currents around the head and neck are avoided. Short wave diathermy is also avoided over metal implants or an intrauterine device, because it heats metal

How many sessions?

Electrotherapy is used in short bursts and should be a shrinking part of your plan, not a constant. Six to ten sessions over two or three weeks is typical, always alongside manual therapy and exercise.

Expect relief lasting from a couple of hours to a day after a session, and a little longer each time if the exercise is being done. If after five or six sessions the relief never outlasts the afternoon, the machine is doing nothing useful and should be stopped.

Repeating a machine for weeks because it feels pleasant is one of the commonest ways physiotherapy gets wasted. If TENS is the part that helps you, a home unit costs little and you can use it yourself.

Urdu

Electrotherapy koi ek ilaj nahi hai. Ye un machinon ka naam hai jo physiotherapy mein chalti hain, jaise TENS, IFT, muscle stimulator, ultrasound aur laser.

Machine par lait jana poora ilaj nahi hai. Machine thori der ke liye dard kam karti hai aur akre hue gosht ko dheela karti hai. Isi waqfe mein warzish aur haath ka ilaj asal faida deta hai.

Machine disc theek nahi karti aur kamzor gosht ko taqat nahi deti. Agar koi clinic sirf bees minute machine laga kar bhej de to wo mukammal ilaj nahi hua.

Jalan ya tez garmi ho to foran batayen. Sun jild par current nahi lagana chahiye. Pacemaker, hamla, kati jild ya kisi gaanth ki jagah par current nahi lagaya jata.

Questions people ask

Is electrotherapy enough on its own?

No, and that is the main thing to understand about it. It reduces pain and relaxes muscle for a few hours. That window is useful, but the change comes from the hands-on treatment and the exercise done inside it. A session that is only machines is incomplete treatment.

Can I have electrotherapy with a pacemaker?

No. Currents are not applied if you have a pacemaker or an implanted defibrillator, and interferential therapy and short wave diathermy are not used at all. Tell Dr. Zahra before any treatment. Manual therapy and exercise are completely safe and are usually the better route anyway.

Does a stronger setting work better?

No. The correct intensity is strong enough to feel clearly and still comfortable. Higher does not mean faster. Burning, stinging or rising heat means something is wrong with the setting or with the pad contact, and continuing risks a skin burn, so say so immediately.

Why do some clinics use machines for everything?

Because machines are quick, they feel like treatment, and many patients expect them. They also take far less of the therapist's time than a proper assessment and a progressed exercise programme. It is reasonable to ask what the machine is for and what is going to follow it.

Can electrotherapy be used over a metal implant?

TENS and muscle stimulation are generally fine over a plate, screws or a joint replacement. Short wave diathermy is not, because it heats metal. Therapeutic ultrasound is not used over a cemented joint replacement. Always tell Dr. Zahra what implant you have and exactly where it is.
