A closer look
| Criterion | Physiotherapy | Surgery |
|---|---|---|
| Purpose | Build movement, function and strength through rehabilitation. | Address a problem where an operation is clinically indicated. |
| When considered | A conservative option for suitable problems after assessment. | When the diagnosis, severity and response to other care justify it. |
| Who decides suitability | A physiotherapist assesses rehabilitation needs and refers when needed. | A surgeon assesses whether an operation is appropriate. |
| Can one replace the other? | Not a substitute for an operation that is medically necessary. | Does not remove the need for appropriate rehabilitation. |
| Relationship | May be used before and after an operation. | Often forms one part of a wider recovery plan. |
The options can complement each other. Your examination determines the appropriate next step.
Where physiotherapy is genuinely tried first: knee osteoarthritis, where exercise and strengthening are the recommended first treatment and replacement is considered when pain and function stay unacceptable despite it. Most sciatica and disc related leg pain, which settles without surgery in the majority of cases. Shoulder tendon pain without a traumatic tear. Frozen shoulder. Tennis elbow, plantar heel pain and Achilles pain, which respond to loading programmes. Mild to moderate carpal tunnel syndrome.
Where surgery is the treatment and physiotherapy is no substitute: displaced fractures and dislocations, a completely ruptured tendon, a knee that is mechanically locked, infection in a joint or bone, a tumour, and nerve compression with weakness that is getting worse. Numbness around the groin or back passage, loss of bladder or bowel control, or both legs weakening is a same day hospital emergency.
Who decides is not in doubt. A physiotherapist cannot tell you whether you need an operation. That needs the examination, the imaging and the judgement of a surgeon. What physiotherapy can do is give the conservative option a fair trial, so that the decision is made on real information rather than guesswork.
Physiotherapy is also part of surgery rather than only an alternative to it. Building strength and movement beforehand improves recovery, and the rehabilitation afterwards is usually what decides the final result. A well done operation with no rehabilitation behind it does not give a good outcome.
What it means for you
If your surgeon has told you an operation is needed, do not use a course of physiotherapy to put it off. If you want to be sure, get a second opinion from another surgeon. A physiotherapist is not the right person to overrule a surgical decision, and no honest one will try.
If surgery is being offered as one option among others, ask the surgeon three questions: what exactly is damaged, what is likely to happen if I do three months of proper physiotherapy first, and is there any risk in waiting. If waiting is safe, a well run trial of treatment is reasonable.
Either way, start the exercises. If you end up avoiding the operation, they are the reason. If you have it, they are what makes the recovery work.
