A closer look
No particular posture has been shown to cause back or neck pain, and plenty of people with a rounded upper back and a forward head have no pain at all. The goal is not a straight line down your side in a photograph. It is being comfortable, and able to change position easily.
What helps is capacity. If your neck and upper back tire after twenty minutes at a desk, you will slump, and reminding yourself to sit up will not change that. Build those muscles and the slump arrives later. Add some mobility work through the upper back, which stiffens in people who sit a lot, and holding the position gets easier again.
The exercises that earn their place are simple: chin tucks, rows or band pull-aparts, extension over a rolled towel laid along the upper back, wall slides, and lying face down lifting the arms in a Y or T shape. Two or three sets, most days, for six to eight weeks before you judge it.
What does not work: a posture brace worn all day, which lets the muscles do less rather than more, one session of having a back clicked to fix posture, apps and gadgets, and holding the shoulders pinned back, which is just another static position.
Some things are structural rather than habit. Scoliosis is a curve in the spine itself, and a fixed rounded upper back in an older adult is often the result of thin bones and small spinal fractures. Exercise helps pain, balance and function in both, but it will not straighten the spine. Be careful of anyone who promises that it will.
What it means for you
- Pick three exercises, not ten. Chin tucks, a rowing movement and extension over a towel cover most of it.
- Do them most days for six to eight weeks. Posture work is strength work and strength takes that long.
- Break up sitting every half hour. That is half the answer on its own.
- Skip the brace. Spend the money on a resistance band instead.
- If pain rather than appearance is the problem, get the pain assessed. Posture exercises are not a diagnosis.
- An upper back that has become visibly rounded over a few months in an older adult, with new back pain, can mean a spinal fracture from thin bones. That needs a doctor.

Barri umar mein kamar achanak jhuk jaye aur dard ho to doctor ko dikhayen.
