How do I build a home exercise routine and stick to it?

A home programme works when it is short, chosen for your problem, and done most days. Three to five exercises in ten to fifteen minutes, five or six days a week, beats an hour-long routine you manage twice. Consistency does more here than content.

A closer look

Home exercise is not a smaller version of treatment, it is most of the treatment. Tissue changes in response to repeated loading over weeks, and two clinic visits cannot provide that. What happens on the other six days decides the result.

Choose the right exercises first. A general routine from a video is aimed at nobody in particular, and the same back exercise that helps one person flares another, because the cause is different. If you do not know what is wrong, get assessed before building a routine. Teaching and correcting exercises is one of the things that works well over video, and that is what the online consultation (fee confirmed on WhatsApp) is for.

Then make it hard to skip. Attach it to something you already do every day, such as morning tea or just before bed. Keep it in the same place with the band or towel already out. Tick it off on a paper calendar on the wall. People who keep going are not more disciplined, they have removed the decisions.

Progress it or it stops working. Every one to two weeks add repetitions first, then sets, then resistance. An exercise that has become easy is no longer treating anything.

Keep a short version for bad days: the two most useful exercises, one set each. Three minutes keeps the habit alive, and the habit is the thing you are protecting. Equipment is almost unnecessary: a wall, a chair, a towel, a filled water bottle and a resistance band from any sports shop cover nearly every programme.

What it means for you

  • Three to five exercises, written on one sheet of paper with the reps and sets on it.
  • Same time, same place, every day. Decide once instead of deciding daily.
  • Progress every week or two. Add repetitions before you add weight.
  • Keep a three minute version for the days you cannot face the full routine.
  • Review at four weeks: better, the same, or worse, then change the plan accordingly.
  • Stop and see a doctor before continuing if you develop new weakness or numbness, fever with back pain, unexplained weight loss, or pain that wakes you every night.

Questions people ask

How often should I do my home exercises?
Most days, five or six out of seven, unless you have been told otherwise. Strength work for one muscle group is often better every other day. Short and frequent beats long and occasional, because tissue needs repeated loading rather than one big session a week.
Can physiotherapy work online?
For teaching and correcting exercises, yes, and that is a large part of treatment. Assessment over video has limits, and hands-on work such as manual therapy, dry needling, ultrasound or TENS cannot be done remotely. Online consultation suits programme design, progression and follow-up.
What equipment do I need for home exercises?
Usually none. A chair, a wall, a towel and a water bottle cover most programmes. A resistance band is the one purchase worth making and costs very little. Machines are not necessary and often get in the way of doing the simple things daily.
I keep forgetting my exercises. What helps?
Shorten the routine until it is almost too easy to refuse, attach it to a daily habit, leave the equipment out where you will trip over it, and tick a paper record. Three exercises done daily are worth more than eight written down and ignored.
