What is muscle stiffness

Muscle stiffness is the feeling that the body has to be unlocked before it will move. The first few minutes are the worst and it eases as you get going. In Urdu it is pathon ki akarhan.

Some stiffness is normal and expected. Sitting still for hours produces it. So does new exercise, which gives soreness and stiffness starting about 24 hours later, peaking around 48 hours and gone within three to five days.

Other stiffness is a message. Stiffness that lasts more than an hour every morning, or that comes with swollen joints, fever or weight loss, is a medical question rather than a physiotherapy one.

Symptoms you might have

  • The body feels locked on waking or after sitting, then loosens over the first few minutes of movement
  • Stiffness worst in the neck, shoulders and lower back in people who sit or work at a machine
  • Soreness and stiffness appearing 24 to 48 hours after unaccustomed exercise
  • Short stiffness after rest that eases with movement, which is the usual pattern in osteoarthritis
  • Morning stiffness lasting more than an hour, with swollen joints, which is not ordinary stiffness

Why it happens

  • Hours held in one position: a desk, a sewing machine, a counter, a motorbike, a long drive
  • A sudden amount of new exercise or unfamiliar heavy work
  • Osteoarthritis, where stiffness is short, worse after rest and eases with movement
  • Inflammatory arthritis, where morning stiffness lasts over an hour and joints swell
  • Low vitamin D, very common in Pakistan, and an underactive thyroid
  • Fibromyalgia, where stiffness comes with widespread pain and heavy fatigue
  • After a stroke, or in Parkinson's disease, where the muscle itself holds tight

When you should see a physiotherapist

Come to physiotherapy for stiffness from posture, long hours of work, inactivity, osteoarthritis, or after an injury or a period in plaster. That covers the large majority, and it responds well to movement and strengthening.

See a doctor rather than a physiotherapist if morning stiffness lasts more than an hour, if joints are swollen, if there is fever or unexplained weight loss, or if severe stiffness in both shoulders and both hips started suddenly after the age of 50. Blood tests sort most of these, and some of them need medicine started promptly.

Also see a doctor if stiffness comes with weakness that is getting worse, or weakness on one side of the body only. Those are nerve and muscle questions and they need naming before they are treated.

How it is treated

For ordinary stiffness, movement is the treatment. Hands-on work and machines make the movement easier and more comfortable, but they do not replace it.

  • Exercise therapy, a short daily routine through full range plus strengthening, because a stronger muscle stiffens less under the same load
  • Manual therapy and joint mobilisation for the areas that have genuinely lost range
  • Hijama (cupping therapy) for broad tightness across the back, shoulders and neck
  • Dry needling for specific tight bands that will not release with hands alone
  • Electrotherapy and TENS where pain is limiting how much you can move
  • Ultrasound over a localised, stubborn area
  • Work setup advice, since a muscle held in one position for eight hours a day will keep protesting

A muscle that grips hard and locks you in one position is a spasm rather than general stiffness, and our muscle spasm page covers that. Surgery has no part in muscle stiffness itself. Where stiffness comes from thyroid trouble, vitamin D deficiency or inflammatory arthritis, the medicine that treats it is a doctor's decision and physiotherapy works alongside it.

Exercises, and what to avoid

What helps

  • A five minute routine in the morning taking the neck, shoulders, back, hips and knees through their full range
  • Getting up and moving every 30 to 40 minutes through the working day
  • Strengthening twice a week for the areas that stiffen most, usually the back, hips and shoulders
  • Walking 20 to 30 minutes most days, which loosens more than any single stretch

What to avoid

  • Relying on stretching alone. It feels good for an hour and changes nothing by evening
  • A week of nothing followed by a weekend of heavy work, which guarantees stiffness and often injury
  • Forcing a joint that is hot and swollen. That needs a doctor, not more movement
  • Bed rest for stiffness, which makes it worse within days

What you can do at home

Heat is the right choice for stiffness. Fifteen to twenty minutes with a hot pack, or a warm shower first thing in the morning, before you try to move.

Ice is for a fresh injury with swelling and bruising, not for stiffness. People reach for the wrong one constantly.

Move before you stiffen rather than after. A minute of movement every half hour beats twenty minutes once a day.

Drink enough water in the hot months. Tight, cramp-prone muscles are often simply dry.

Sleep matters more than people expect. Several short nights in a row show up as stiffness and aching the next week.

How long it takes

Stiffness and soreness after new exercise peaks at about 48 hours and is gone within three to five days. It is normal and it needs no treatment beyond gentle movement.

Stiffness from posture and long working hours usually improves within two to four weeks of movement breaks and strengthening, provided the work setup actually changes.

Osteoarthritis stiffness responds to six to eight weeks of strengthening, and it keeps responding as long as the exercise continues. Stop the exercise and it comes back within a couple of months.

Stiffness caused by low vitamin D or an underactive thyroid improves once the doctor treats the cause, usually over six to twelve weeks. No amount of physiotherapy substitutes for that.

Questions people ask

Why am I so stiff in the mornings?
Short stiffness that eases within ten or fifteen minutes of getting going is normal, and it is the usual pattern with osteoarthritis and with long hours in one position. Stiffness that lasts more than an hour every morning, especially with swollen joints, needs a doctor and blood tests.
Is stiffness after exercise a bad sign?
No. Soreness and stiffness that start about 24 hours after unfamiliar exercise, peak around 48 hours and clear within three to five days are normal. Gentle movement helps it pass. Sharp pain during the exercise itself is a different thing and worth having checked.
Heat or ice for stiffness?
Heat. Fifteen to twenty minutes with a hot pack, or a warm shower before you try to move in the morning. Ice is for a fresh injury where there is swelling or bruising. Using ice on stiffness is one of the commonest mistakes people make.
Will stretching fix my stiffness?
Stretching alone gives an hour of relief and changes little by evening. What changes stiffness is moving regularly through the day and strengthening the areas that stiffen. A stronger muscle holds the same load with less tightness, which is why strengthening outlasts stretching.
Can vitamin D deficiency cause stiffness?
Yes, and it is very common in Pakistan. Low vitamin D causes aching and stiffness, often in the thighs, back and legs, along with difficulty rising from the floor. The test and the treatment are your doctor's, and physiotherapy works alongside them.
