A closer look
There is no single correct position. The best position is the one that lets you sleep, because broken sleep makes pain worse the next day, and that matters more than any rule about alignment.
On your side, a pillow between the knees stops the top leg dragging the pelvis forward and twisting the lower back. The pillow under your head should fill the gap between your shoulder and your ear so the neck stays in line with the rest of you, not propped high and not left flat. If one side is clearly worse, lie on the side that hurts less.
On your back, one or two pillows under the knees take the pull off the lower back. A small rolled towel in the curve of the lower back suits some people and not others, so try it rather than assuming.
Face down flattens the lower back into an arched position and forces the neck to stay turned for hours. If you genuinely cannot sleep any other way, put a thin pillow under your lower belly and hips and use a flat pillow, or none, under your head.
The bed matters less than people expect. Medium firm suits most backs. A sagging mattress or a charpai with a dip in the middle is a real problem, because the hips sink and the spine stays twisted all night. A board under a sagging mattress is a cheap fix that often works better than a new mattress.
What it means for you
- Side lying, knees bent, pillow between the knees. Most people need nothing more than this.
- Get the head pillow height right so your neck stays level with your spine. One good pillow, not three.
- Fix the bed before you buy anything else. A board under a sag costs almost nothing.
- Get out of bed by rolling onto your side, dropping your legs over the edge, then pushing up with your arms. Do not sit straight up from flat.
- Give any new position three or four nights before judging it. The first night in a new position is always worse.
- Back pain that is clearly worse lying down, wakes you every night, and comes with unexplained weight loss or fever needs a doctor, not a new pillow.
