Can physiotherapy help me avoid surgery?

Physiotherapy can be an alternative to surgery for some problems, but it cannot replace an operation that is medically necessary. For several common problems, including knee osteoarthritis, most sciatica and shoulder tendon pain, exercise based treatment is a recommended first option and many people never need an operation. For others, surgery is the treatment and delay causes harm. The decision belongs to the surgeon who has examined you and seen the imaging.

Physiotherapy and surgery: different roles in care
CriterionPhysiotherapySurgery
PurposeBuild movement, function and strength through rehabilitation.Address a problem where an operation is clinically indicated.
When consideredA conservative option for suitable problems after assessment. When the diagnosis, severity and response to other care justify it.
Who decides suitabilityA physiotherapist assesses rehabilitation needs and refers when needed.A surgeon assesses whether an operation is appropriate.
Can one replace the other? Not a substitute for an operation that is medically necessary.Does not remove the need for appropriate rehabilitation.
RelationshipMay be used before and after an operation.Often forms one part of a wider recovery plan.

The options can complement each other. Your examination determines the appropriate next step.

Where physiotherapy is genuinely tried first: knee osteoarthritis, where exercise and strengthening are the recommended first treatment and replacement is considered when pain and function stay unacceptable despite it. Most sciatica and disc related leg pain, which settles without surgery in the majority of cases. Shoulder tendon pain without a traumatic tear. Frozen shoulder. Tennis elbow, plantar heel pain and Achilles pain, which respond to loading programmes. Mild to moderate carpal tunnel syndrome.

Where surgery is the treatment and physiotherapy is no substitute: displaced fractures and dislocations, a completely ruptured tendon, a knee that is mechanically locked, infection in a joint or bone, a tumour, and nerve compression with weakness that is getting worse. Numbness around the groin or back passage, loss of bladder or bowel control, or both legs weakening is a same day hospital emergency.

Who decides is not in doubt. A physiotherapist cannot tell you whether you need an operation. That needs the examination, the imaging and the judgement of a surgeon. What physiotherapy can do is give the conservative option a fair trial, so that the decision is made on real information rather than guesswork.

Physiotherapy is also part of surgery rather than only an alternative to it. Building strength and movement beforehand improves recovery, and the rehabilitation afterwards is usually what decides the final result. A well done operation with no rehabilitation behind it does not give a good outcome.

What it means for you

If your surgeon has told you an operation is needed, do not use a course of physiotherapy to put it off. If you want to be sure, get a second opinion from another surgeon. A physiotherapist is not the right person to overrule a surgical decision, and no honest one will try.

If surgery is being offered as one option among others, ask the surgeon three questions: what exactly is damaged, what is likely to happen if I do three months of proper physiotherapy first, and is there any risk in waiting. If waiting is safe, a well run trial of treatment is reasonable.

Either way, start the exercises. If you end up avoiding the operation, they are the reason. If you have it, they are what makes the recovery work.

Questions people ask

Can physiotherapy fix a slipped disc without surgery?
It cannot push a disc back, but most disc related leg pain settles without surgery, with time, the right exercise and changes to activity. Surgery is considered when pain stays severe after some months, or when leg weakness is getting worse. Bladder or bowel changes need urgent hospital assessment.
Should I try physiotherapy before a knee replacement?
Usually yes, if your surgeon agrees there is no urgency. Exercise and strengthening are the recommended first treatment for knee osteoarthritis and can postpone replacement in some people. If the joint is badly destroyed and function is gone, physiotherapy will not change that.
Is it dangerous to delay surgery and try physiotherapy?
It depends entirely on what is wrong, which is why the surgeon must answer it. For some problems, waiting costs nothing. For a fracture, a locked knee, an infection, a complete rupture or worsening nerve weakness, waiting causes lasting damage. Ask directly whether delay is safe.
Do I need physiotherapy after surgery?
For most orthopaedic operations yes, and it is usually what decides the outcome. Follow the protocol your surgeon gives you, because early movement limits differ by procedure. Physiotherapy then restores range, strength and normal walking within those limits.
