What is back pain while sitting

Back pain that comes on when you sit and eases when you stand is one of the commonest complaints we see. In Urdu it is baithne se kamar mein dard.

Sitting is harder on the lower back than standing. In a slumped position the back is held in a bend, the discs and joints take a steady load, the trunk muscles switch off, and nothing moves, so no part of the back gets a rest.

The pattern itself is a clue. Pain that starts after 10 to 20 minutes of sitting and clears within a minute of walking usually means a mechanical back that does not like sustained bending. Pain relieved by sitting and brought on by standing is a different problem, covered on our spinal stenosis page.

Symptoms you might have

  • Ache across the lower back that builds the longer you sit
  • Relief within a minute or two of standing up and walking
  • Worse after a long motorbike ride, which is sitting plus constant jarring
  • Pain right at the very bottom of the spine when sitting points to the tailbone instead, covered on our tailbone pain page
  • Pain in one buttock when sitting on that side points to the SI joint
  • Pain going below the knee with pins and needles means a nerve is involved, which is sciatica

Why it happens

  • Hours at a desk or a shop counter without standing up
  • Chairs with no back support, plastic chairs and sagging sofas
  • Motorbike riding, which combines a held position with constant vibration
  • Sitting cross legged on the floor for meals and household work, which rounds the lower back fully
  • Tailoring, embroidery and other work bent forward over the hands for hours
  • Weak trunk and hip muscles, which make upright sitting tiring within minutes

When you should see a physiotherapist

Come in if sitting pain is affecting your work, if it has lasted more than two or three weeks, or if pain has started travelling into a leg. These cases usually respond quickly once the pattern is identified, because there is normally one direction of movement that eases it.

Go to a hospital the same day with numbness around the groin or back passage, loss of bladder or bowel control, or weakness in both legs. Pay particular attention to numbness in the area you sit on.

See a doctor rather than a physiotherapist for back pain with fever, unexplained weight loss, night pain that does not change with position, or after a significant fall, and promptly if a leg is getting weaker.

How it is treated

The assessment finds two things: which movement direction eases your pain, and what in your day keeps feeding it. The treatment follows from both.

  • Exercise therapy, starting with repeated movement in the easing direction, which for most sitting related pain is gentle extension, then trunk and hip strengthening
  • Manual therapy to the stiff lower back segments and hips, because stiff hips force the lower back to bend further when you sit
  • Sitting setup, done properly once rather than guessed at: chair height, back support, screen height, and what to put behind your belt line
  • Dry needling for the muscles beside the spine stuck in spasm
  • TENS and electrotherapy for pain relief in the first week or two
  • Motorbike and driving advice, which matters more here than anywhere else in Pakistan

Surgery has no role in ordinary sitting related back pain. If a nerve is involved, that is assessed separately and the decision belongs to a spine surgeon.

Exercises, and what to avoid

What helps

  • Standing up and walking for two minutes every 20 to 30 minutes. This is the treatment, not a break from it
  • Lying face down and propping on the elbows for a few minutes, if that eases your pain
  • Hip flexor and hamstring flexibility work, because both tighten with long sitting
  • Glute bridges, bird dog and side planks, plus practising sit to stand without using your hands

What to avoid

  • Sitting through the pain to finish the task. The pain climbs and takes longer to settle afterwards
  • Sit ups, crunches and toe touching while the back is sore
  • Long stretches sitting cross legged on the floor
  • Buying a chair or a cushion instead of changing how often you move. No chair fixes six unbroken hours

What you can do at home

Set up a sitting position you can hold without effort: hips level with or slightly above the knees, feet flat, back supported, and a rolled towel at your belt line.

Stand up every 20 to 30 minutes. Set an alarm for the first week. This habit outperforms any chair you can buy.

On a motorbike, stop and stand for a minute on long rides, and raise the handlebars if you can.

For floor sitting, put a firm cushion under your buttocks so your hips sit higher than your knees, lean against a wall, and change your legs often.

Heat suits this best, 15 to 20 minutes on the lower back after a long sitting day. Ice only for a fresh sharp strain.

How long it takes

Most sitting related back pain improves within two to four weeks once the pattern is identified and the sitting changes, and many people feel a difference inside the first week.

If the job cannot change much, expect six to twelve weeks, because the work shifts to building enough trunk and hip strength that long sitting stops hurting.

Pain that has been there for over three months takes three to six months, and the strengthening matters more than the setup at that stage.

What keeps it going is sitting until the task is finished. Standing up on a timer is the change that most often breaks the cycle.

Questions people ask

Why does my back hurt when I sit but not when I stand?
Because slumped sitting holds the lower back in a bend and loads the same structures continuously, while standing lets you shift your weight constantly. If the opposite is true for you, and standing hurts while sitting relieves it, that points to spinal stenosis.
What is the best chair for back pain?
Less important than people hope. A firm seat with back support, at a height that puts your hips level with or slightly above your knees, is enough. A rolled towel at your belt line does what most lumbar supports do. Getting up every half hour matters far more.
Is sitting on the floor bad for my back?
It is harder on the lower back than a chair, because cross legged sitting rounds the spine fully and holds it there. If you prefer the floor, raise your hips with a firm cushion, use a wall for support, change legs often, and get up regularly.
Why does my back hurt after riding a motorbike?
Because a motorbike combines three problems: a held forward position, no back support, and continuous vibration through the spine from broken roads. For many patients here it is the single biggest cause. Breaking up long rides and raising the handlebars both help.
Should I use a back belt at my desk?
Not routinely. Worn all day for months, a belt means the muscles do less and they get weaker, so you end up relying on it. Occasional use for a heavy task is reasonable. Strength and standing up regularly are better and cost nothing.
