What is disc bulge

A disc bulge is the outer ring of a disc pushing out slightly beyond its normal edge, across a broad area. A herniation is different: the soft centre pushes through a tear at one spot and can press on a nerve. Our slip disc page covers that.

Now the word itself. In Urdu people read the report and say disc nikal gayi hai, the disc has come out. It has not.

Disc bulges are found on MRI in roughly a third of people in their twenties with no back pain at all, and in most people by their sixties. They are part of ordinary ageing, like grey hair. Finding one does not tell you it is the cause of your pain, and it does not mean your back is fragile.

Symptoms you might have

  • Often nothing at all, which is the most important line on this page
  • When it is causing symptoms: lower back ache across the belt line, worse with sitting and bending forward
  • Pain spreading into the buttock or the back of the thigh
  • No numbness and no leg weakness in ordinary disc related back pain
  • Pain travelling below the knee with pins and needles means a nerve is involved, which is more typical of a herniation. Our sciatica page covers that

Why it happens

  • Normal ageing of the disc, which starts in the twenties in most people
  • Family history, which has a bigger influence on disc wear than most people expect
  • Lifting with a bent and twisted back
  • Long hours sitting with no back support, at a desk, a counter or on a motorbike
  • Heavy manual work, loading and carrying day after day
  • Smoking, which reduces the blood supply that feeds a disc
  • Repeated bending and squatting during household or field work

When you should see a physiotherapist

Come in if the back pain is limiting you, whatever the report says. We treat the back and the person, not the film.

If you have a scan showing a bulge and no symptoms, you do not need treatment at all. You need reassurance and a reason to keep your back strong.

Go to a hospital the same day with numbness around the groin or back passage, loss of bladder or bowel control, or weakness in both legs.

See a doctor promptly if a leg is getting weaker, if your foot catches on the ground when walking, or if numbness is spreading. See a doctor rather than us for back pain with fever, unexplained weight loss, night pain that does not change with position, or after a bad fall.

How it is treated

There is no technique that pushes a bulge back in, and anyone who tells you otherwise is guessing. What works is settling the irritation, restoring movement, and building a back that takes load without complaining.

  • Exercise therapy, the main treatment. Movement in the direction that eases your pain first, then trunk, hip and leg strengthening
  • Manual therapy to free the stiff segments and the hips, which makes the exercises easier and less painful
  • Dry needling for the muscles beside the spine stuck in protective spasm
  • Hijama (cupping therapy) where the tightness is spread across the lower back
  • Electrotherapy and TENS for pain control in the first couple of weeks
  • Explanation, which is genuine treatment here. Fear of the word bulge keeps people still, and staying still is what makes backs worse

Surgery has no role for a disc bulge without nerve compression. Operating on a scan finding rather than on symptoms is how people end up worse than they started, and any surgical decision belongs to a spine surgeon.

Exercises, and what to avoid

What helps

  • Walking daily, starting with ten minutes a few times a day
  • Gentle movement in both directions: knee rolls, pelvic tilts, and lying face down propped on the elbows
  • Glute bridges, bird dog and side planks once the sharp pain has eased
  • Hip and hamstring flexibility, because stiff hips push extra load into the lower back

What to avoid

  • Avoiding movement because of what the report said. This is the commonest mistake after an MRI
  • Repeated forward bending, toe touching and sit ups while the back is sore
  • Lifting with a rounded twisted back, and carrying loads away from your body
  • Waiting for a repeat scan before you start moving. The bulge does not need to change for your pain to go

What you can do at home

Keep moving. Carrying on with normal activity in a modified way beats resting, and a day in bed is already a day too many.

Break up sitting every 20 to 30 minutes. Riding a motorbike counts as sitting, and for many people here it is what keeps the back pain going.

Heat suits this better than ice, because the muscles beside the spine tighten protectively. Fifteen to twenty minutes of a hot pack. Ice is only for the first day or two after a sudden sharp strain.

When you lift, bend the knees, keep the load close, and do not twist while carrying.

How long it takes

Back pain with a bulge on the scan behaves like ordinary mechanical back pain. Most episodes improve a great deal within two to six weeks, and you should feel some change in the first week or two of treatment.

Pain that has been there more than three months takes three to six months of consistent work, mostly strengthening.

The bulge itself does not have to change for the pain to go, and most people never find out whether it did, because a repeat scan is rarely useful.

Recurrence is common and it is not a sign your spine is crumbling. It is the reason the strengthening is worth continuing after the pain has gone.

Questions people ask

Is a disc bulge serious?
Usually not. Bulges are found on MRI in about a third of people in their twenties with no pain at all, and in most older adults. It becomes a medical problem only when it presses on a nerve and causes leg symptoms or weakness.
What is the difference between a disc bulge and a slip disc?
A bulge is the outer ring pushing out evenly across a broad area. A herniation, which people call a slip disc, is the soft centre pushing through a tear at one spot, and that is far more likely to touch a nerve.
Can a disc bulge be reversed or pushed back?
No treatment pushes a bulge back in, and nobody can click it into place. Treatment settles the irritation, restores movement and builds strength so your back tolerates load again. People get completely better without the bulge changing at all.
Do I need surgery for a disc bulge?
Almost never. Surgery is for nerve compression with worsening weakness, or urgently for loss of bladder or bowel control. Operating because a scan shows a bulge, when the symptoms do not match it, is how people end up worse.
Should I stop exercising if my MRI shows a bulge?
No, and this is the most damaging thing people take from a report. Backs get worse with inactivity, not better. The exercise may need adjusting while the pain is sharp, but the long term answer for a bulge is a strong, well used back.
