What is bad posture

Posture is simply how you hold yourself. People usually come in wanting it fixed after seeing a photograph of themselves, and the first honest thing to say is that there is no single correct posture.

Researchers have looked hard for a link between the shape of a person's spine and whether they have pain, and it is far weaker than most people expect. Plenty of people with rounded shoulders and a forward head have no pain at all, and plenty who sit beautifully have back pain.

What does cause pain is holding one position for hours, having muscles too weak to hold you up comfortably, and never varying what you do. So the useful rule is this: the best posture is the next one.

Symptoms you might have

  • Ache that builds through the day and eases when you move about or lie down
  • A tight band across the top of the shoulders and between the shoulder blades
  • Lower back ache after an hour of sitting, relieved by standing up
  • Neck ache and headache at the base of the skull after screen work, covered on our neck pain page
  • Pain that does not fit this pattern, for example waking you at night or travelling down a leg, is something else and needs assessing properly

Why it happens

  • Sitting or standing in one position for hours with no break, which is the real culprit
  • Weak trunk, hip and shoulder blade muscles, so holding yourself up is tiring
  • Low screens, laptops flat on tables, and phones held down at the waist
  • Close hand work: tailoring, embroidery, stitching, repair work, dental and laboratory work
  • Poor sleep and stress, both of which increase how sensitive the body is to ordinary load
  • A structural cause such as scoliosis, Scheuermann's kyphosis or a vertebral fracture, which is a different situation. Our scoliosis and kyphosis pages cover those

When you should see a physiotherapist

Come in if posture related pain is a daily thing, if your job keeps you in one position, or if internet stretches have not changed anything in months.

What posture work can do: reduce pain, build endurance so sitting and standing stop being tiring, free up stiff areas, and make your working day fit you. What it cannot do: change the shape of your bones, or guarantee you never get back pain again.

Go to a hospital the same day with numbness around the groin or back passage, loss of bladder or bowel control, or weakness in both legs.

See a doctor if your posture has visibly changed over months, and especially for a newly rounded upper back with pain in an older adult, which can mean a vertebra has fractured. Also for back pain with fever, unexplained weight loss, or leg weakness that is getting worse.

How it is treated

Treatment is aimed at the two things that genuinely matter: how long you stay in one position, and whether your muscles can comfortably hold you there.

  • Exercise therapy, the centre of it. Strength and endurance for the neck, shoulder blade, trunk and hip muscles, because a tired muscle is what makes good positions unsustainable
  • Movement variety training, which means building position changes into your day instead of trying to hold one shape
  • Manual therapy for genuinely stiff segments, usually the upper back and hips, which makes a taller position easier to reach
  • Dry needling for painful tight points across the shoulders and beside the spine
  • Hijama (cupping therapy) for a widely tight upper back
  • TENS and electrotherapy for pain relief, so the strengthening can actually be done
  • Workstation and work setup advice, which usually delivers more than any exercise

There is no surgery for posture. There is also no belt, no device and no single exercise that fixes it, whatever the advertisements claim.

Exercises, and what to avoid

What helps

  • Upper back extension over a rolled towel, and rotation, done daily
  • Band rows, face pulls and shoulder blade squeezes, two or three times a week with real resistance
  • Trunk endurance: side plank, bird dog and dead bug, built up over weeks rather than days
  • Hip flexor and hamstring flexibility, plus chin tucks for the neck

What to avoid

  • Posture correction belts and braces worn all day. They hold you up while your muscles do less, and over months you need them more
  • Trying to hold one perfect posture. It is tiring, nobody sustains it, and it is not the goal
  • Stretching only. The stretch feels good for twenty minutes and the ache comes back by evening
  • Expecting ten minutes of exercise to undo nine hours in one chair

What you can do at home

Change position every 30 minutes. If you take one thing from this page, take that one.

Set the screen top at eye level, bring the keyboard close, and hold the phone up at your face.

Do two short strengthening sessions a week. Twenty minutes each is enough, and it is the part that stops the ache returning.

Heat helps the muscle ache, 15 to 20 minutes on the sore area. Ice is rarely the right choice for posture related pain.

Protect your sleep. Pain tolerance drops sharply after a run of bad nights, and the same desk suddenly hurts more.

How long it takes

The ache usually improves within two to four weeks, mostly from changing how long you stay in one position rather than from any exercise.

Strength and endurance take longer. Expect eight to twelve weeks before sitting and standing tall stop feeling like work.

Visible change in how you look is slower and only partial. A flexible rounded back improves noticeably over three months. A structural curve does not change shape, although it can feel much better.

This one has no finish line. The people who keep two sessions a week and keep moving through the day are the ones who stay comfortable.

Sach ye hai ke koi ek sahi posture nahi hoti. Reerh ki haddi ki shakal aur dard ka taalluq itna mazboot nahi jitna log samajhte hain.

Asal masla ye hai ke insan ghanton ek hi position mein rehta hai aur pathe is kaam ke liye kamzor hote hain. Behtar usool: har 30 minute baad position badlen.

Ilaj: taqat aur stamina ki warzish, manual therapy, stretching, dry needling aur hijama. Screen aankhon ki sidh mein aur mobile oopar rakhen.

Doctor ko dikhayen agar bari umar mein kamar naye tor par gol ho gayi ho aur dard bhi ho. Foran hospital jayen agar sharamgah ya pakhane ke gird sun hona ho, peshab pakhane par control khatam ho jaye, ya dono taangein kamzor hon.

Questions people ask

Is bad posture really causing my pain?
Partly, but not in the way most people think. The shape you hold predicts pain less well than how long you hold it and how strong you are. Hours without moving, and muscles that fatigue quickly, are what produce the ache. Both are fixable.
What is the correct posture for sitting?
There is no single correct one. A reasonable starting position is hips level with or slightly above the knees, feet flat, back supported and screen at eye level. More important is leaving that position every half hour. The best posture is the next one.
Do posture correctors and belts work?
Not for lasting change. A belt holds you upright while you wear it, your muscles work less, and after a few months you are weaker underneath it. Occasional use for comfort does no harm. Strength and movement variety are what hold a better position.
Can posture be corrected at any age?
Comfort, strength and endurance improve at any age, often a great deal. How much the shape changes depends on whether the rounding is habit or bone. A flexible posture improves visibly. A structural curve keeps its shape and still feels much better.
How long before I see a difference?
The ache often eases within two to four weeks, mostly from moving more often during the day rather than from any exercise. Strength and endurance take eight to twelve weeks. Visible change in appearance is slower, around three months, and it is partial.
