What is text neck

Text neck is the ache at the back and base of the neck that comes from long hours looking down at a phone or a laptop. Nothing in your neck is damaged by it. It is a loading problem: muscles holding one position for hours get sore.

In Urdu it is simply gardan ka dard, and the usual complaint is that it builds through the day and eases overnight.

One honest point, because it is sold the other way: the research does not show that one perfect posture prevents neck pain. Sitting straighter for eight hours without moving still hurts. How long you stay in any position, and how strong your neck and upper back are, matter more than the exact angle.

Symptoms you might have

  • An ache at the base of the neck and across the tops of the shoulders
  • Pain mild in the morning, building through the day, worst by evening
  • Headache starting at the base of the skull
  • Better on a day off, which is the clearest sign it is load and not damage
  • No numbness or weakness in the arm. If the arm is numb or weak, that is a nerve problem and the cervical radiculopathy page covers it

Why it happens

  • Hours on a phone with the head bent forward, which multiplies the load the neck muscles hold
  • Staying still, which matters more than the exact angle. Any position held for hours becomes painful
  • Weak deep neck and shoulder blade muscles, the most treatable cause of all
  • Long motorbike rides, where the neck is held in one position and absorbs the jolting
  • Studying for exams, tailoring, embroidery and other close work done for long stretches
  • Poor sleep and high stress, which lower the tolerance of every muscle in the area

When you should see a physiotherapist

Come in when the pain is there most days, when it wakes you, or when it is not settling after a few weeks of your own efforts. It responds well, and it is worth getting the strengthening right rather than collecting stretches from the internet.

See a doctor rather than a physiotherapist if the neck pain followed an accident or a fall, if there is fever, night sweats or unexplained weight loss, if the pain is severe and constant day and night, or if it began with a sudden very severe headache and a stiff neck.

See a doctor urgently if your arm is becoming weak, if your hands have become clumsy or you are dropping things, if your walking is unsteady, or if there is any change in bladder or bowel control.

How it is treated

The assessment checks neck and upper back movement, the strength of the deep neck and shoulder blade muscles, and how your day is set up. Treatment has two halves: settle the pain now, and build the capacity so the same day stops hurting.

  • Manual therapy to the neck and upper back, which gives fast relief from stiffness and headaches and makes the exercise easier to do
  • Exercise therapy, deep neck flexor training and shoulder blade strengthening. This is the part that changes the outcome, and it takes five minutes a day
  • Upper back mobility work, because a stiff mid back forces the neck to do more
  • Dry needling for tight, tender bands in the shoulder and neck muscles, which often unlocks a stubborn case
  • Electrotherapy and ultrasound for pain and spasm in a bad flare up

There is no surgery for this and it does not need scans in the usual case. An X ray of a neck like this often shows age related changes that are present in people with no pain at all.

Exercises, and what to avoid

What helps

  • Chin tucks: chin drawn gently back and level, hold five seconds, ten times, two or three times a day
  • Shoulder blade squeezes and band rows, which build the support the neck sits on
  • Upper back extension over a chair back or a rolled towel along the spine, two minutes
  • Neck rotation and side bending to the end of comfortable range, little and often through the day

What to avoid

  • Trying to hold a perfect posture all day. It is tiring, it does not work, and moving often beats sitting still correctly
  • Rolling the head in full circles, and forcing a stretch until it hurts
  • Only stretching. Stretching feels good for twenty minutes, strengthening is what lasts
  • Letting anyone forcefully crack your neck, especially with arm numbness or dizziness

What you can do at home

Raise the screen to eye level. A laptop on a few books with a separate keyboard costs nothing and fixes most of a desk setup. A laptop on the floor or on a bed is the worst arrangement there is.

Hold the phone up rather than dropping the head to it. Rest that elbow on a cushion so you can keep holding it there.

Set a timer for every thirty minutes. Stand, roll the shoulders, turn the head both ways, sit back down. Movement variety is the whole treatment in one habit.

Heat for fifteen to twenty minutes on the neck and upper back at the end of the day. Ice is only useful in the first day or two of a sharp flare up.

How long it takes

With treatment and a changed setup, most people feel clearly better within one to two weeks and settle over four to six weeks.

Pain that has been there for years takes longer, usually eight to twelve weeks, because the strength has to be built rather than borrowed. Five minutes a day is enough if it is daily, and six weeks is a fair trial before you judge it.

It comes back if the daily load returns and the exercises stop. Keeping the screen up, moving every half hour and two short exercises is what holds it. Exam season, a new job and long travel are the usual times it returns.

Questions people ask

Is text neck permanent damage?
No. Nothing in the neck is damaged by looking at a phone. It is muscles and joints complaining about a position held too long, which is why it eases on a day off. Strength and movement breaks reverse it in most people within four to six weeks.
Does bad posture cause neck pain?
Partly, but it is oversold. The evidence does not support the idea that one perfect posture prevents pain, and sitting straighter without moving still hurts. How long you stay in any position, and how strong your neck and upper back are, matter more.
What is the best exercise for text neck?
Chin tucks and shoulder blade squeezes, done daily. Chin tucks train the deep muscles that hold the head up, and shoulder blade work builds the base the neck sits on. Five minutes a day beats a long session once a week.
Do I need an X ray or MRI for neck pain?
Usually not. Ordinary neck pain with no arm numbness, no weakness and no injury does not need imaging, and a neck X ray often shows age related changes also present in people with no pain.
Will a neck collar or a special pillow fix it?
A collar is not helpful for this and wearing one weakens the neck further. A pillow that supports the gap between your head and the mattress helps you sleep, which is worth having, but no pillow strengthens anything. The exercise does that.
